Can AI Teach Us to Be Better at Roleplaying Patient Scenarios in Healthcare? I Built a Prototype to Help Dentists. Let’s Find Out.

Let’s be honest: nobody likes going to the dentist.

That simple truth creates a huge challenge for dental practices. How do you make patients feel comfortable, educate them on necessary treatments, and discuss costs without adding to their anxiety? It’s a delicate conversation that requires a unique blend of empathy, clarity, and sales acumen.

And its true in all healthcare settings as well.

So, can AI help?

A while back, a well-known dental trainer approached me with a fascinating idea. He asked, “Can we use AI to create realistic role-playing scenarios for dentists and their staff? Can we give them a safe space to practice difficult conversations and get better at serving their patients?”

I loved the challenge. At OnStak, where I’m the Chief AI Architect, we’ve built over 115 AI applications this year alone, but this was a unique opportunity to blend technology with a very human skill.

So, we built a rapid prototype. Here’s a look at what we created and what it reveals about the future of dental and healthcare training.

Introducing the “Dental Trainer”

The goal was to create an application where dental professionals could log in and practice real-world patient conversations.

We started by building a library of common Practice Scenarios, each with a different difficulty level. For example:

  • Discussing a sleep apnea appliance for a patient who can’t tolerate a CPAP machine.
  • Addressing a skeptical patient’s concerns about the cost and necessity of cosmetic veneers.
  • Educating a patient on the benefits of professional teeth whitening versus over-the-counter strips.

When a user selects a scenario, they’re presented with a detailed patient profile. For instance, they might meet “Madison,” a 29-year-old social media manager who is unhappy with coffee stains but is worried about tooth sensitivity and cost.

The Magic: AI-Powered Role-Playing

This is where the system comes to life. The real power is in the Voice Mode, where the user can have a real-time, spoken conversation with the AI-powered patient.

I tested it myself, playing the role of the dentist. I greeted “Madison,” acknowledged her concerns, and explained our practice’s approach.

The AI’s response was stunningly realistic. It replied:

“That sounds promising, but I’m still a bit concerned about sensitivity… Can you tell me more about how these organic products work? And how does the cost compare to over-the-counter products?”

The conversation continued, with the AI asking intelligent follow-up questions about payment plans, the number of treatments needed, and how long the results would last. It wasn’t just a script; it was a dynamic, interactive dialogue that mirrored a real patient consultation.

The Feedback Loop: AI as a Coach

Practicing is great, but improvement requires feedback.

After each role-playing session, the user can click “End Conversation” and receive instant, AI-generated feedback. Our system analyzes the entire conversation and provides a performance score across several key areas:

  • Empathy and Rapport
  • Clarity and Communication
  • Persuasion and Value Proposition
  • Objection Handling

It highlights your strengths (e.g., “The dentist showed an understanding of Madison’s concerns”) and identifies areas for improvement (e.g., “The dentist could have been more persuasive in presenting the value…”). It even suggests alternative responses to help you improve for next time.

Over time, users can track their progress on a personal dashboard, watching their scores improve as they practice.

Why This Matters Beyond Dentistry

This prototype is more than just a tool for dentists. It’s a powerful proof of concept for how AI can revolutionize training in any industry that relies on human interaction.

Imagine:

  • Onboarding new sales reps by letting them practice handling objections before they ever speak to a real customer.
  • Training financial advisors on how to navigate difficult conversations about market volatility.
  • Gamifying online courses by incorporating interactive, role-playing modules that bring theoretical concepts to life.

AI is going to be in everything we do. Using it the right way—to empower and elevate human skills—is where the real value lies.
